Consent to Treatment

1. Consent to Treatment: I voluntarily consent to receive healthcare services from the Nurse Practitioner and healthcare team. I understand that this includes routine diagnostic procedures, treatments, and examinations deemed necessary.

2. Medical Information: I authorize the healthcare team to obtain my medical history and records from other healthcare providers as needed for my care.

3. Risks and Benefits: I understand that all medical treatment involves risks, and I have been given the opportunity to discuss these with my healthcare provider.

4. No Guarantees: I understand that no guarantees have been made regarding the outcome of any examination or treatment.

5. Financial Responsibility: I understand that I am financially responsible for all charges regardless of insurance coverage. Payment is due at the time of service unless other arrangements have been made.

6. Cancellation Policy: I understand that I must provide at least 24 hours notice to cancel or reschedule an appointment, or I may be charged a cancellation fee.
